Add shrimp in a single layer (cook in batches if needed). Cook 1-2 minutes per side until pink and opaque. Immediately remove from pan; set aside.

    Bring a large pot of heavily salted water to a vigorous boil. Add fettuccine; stir gently. Cook according to package directions until al dente. Before draining, reserve 1 to 1 ½ cups of starchy pasta water. Drain pasta (do not rinse) and set aside.

    In the same skillet (or a clean one), melt ½ cup unsalted butter over medium heat. Add 4 cloves minced garlic; sauté 1 minute until fragrant (do not burn). Pour in 2 cups heavy cream; gently simmer 2-3 minutes over medium-low heat, stirring occasionally, until slightly thickened.

    Reduce heat to lowest. Gradually whisk in 2 cups freshly grated Parmesan until completely melted and smooth. Stir in ¼ tsp freshly grated nutmeg. Season with salt and fresh black pepper to taste (Parmesan is salty, add incrementally). If sauce is too thick, whisk in reserved pasta water, 1-2 tbsp at a time, until desired glossy consistency.

    Add drained fettuccine directly to the warm Alfredo sauce; toss gently to coat. Gently fold in the cooked shrimp. Squeeze 1 tbsp fresh lemon juice over the dish and sprinkle in half of the chopped fresh parsley. Toss once more. Taste and adjust seasonings. Serve immediately, garnished with remaining parsley, extra Parmesan, and black pepper. Optional lemon wedges for individual servings.
